Release of Information Form:

You must register your student at the Pelham Elementary School main office. You will need to complete the Registration packet along with the Release of Information Form if you are transferring your student from another school. Both forms can be found linked above on this page. You will need to bring these forms with you along with the following items in order for your student’s registration to be considered complete.
- Student registrations are accepted during normal school office hours.
- A birth certificate for the student must be presented. Parents will be asked to provide (2) Proofs of Residency for the Town of Pelham (please see Residency Affidavit in the packet for examples) and sign our Residency Affidavit.
- Up-to-Date Immunization Records
- Parents must provide documentation of a physical exam, by a health care provider, dated within one year of the student's starting date in the Pelham School District.
