Immunization Information
All students need a TdaP (tetanus/pertussis) booster if they are going into 7th grade by the first day of school, if this is not done your child may be excluded from school, this is for your child's safety as well as the safety of other children in the building.
Please send in updated immunization information as soon as possible, as well as physicals. If you have an appointment after the first day of school please let me know.
Children can be excluded from school, per the state, if immunizations are not up to date, there is no appointment date set up and there is no record of this in the school. If there is a problem you can contact the state department of immunizations and vaccinations or check on their web site: at

. The only way a child is excused is by a religious or medical exemption, that has been notarized and is on file in the child's record.
All immunizations must be in English and include full dates.
Requirements do change and different states have different recommendations.
