NOTICE TO PARENTS OF PHS STUDENTS
Please notify the school nurse at 635-6906 and send in a copy of any immunization your child receives so she can keep the school health record updated. Also, please keep the nurse updated on any changes or important health information regarding your child's health. Thank you for your attention to this matter.

FREE FLU SHOTS AND PHYSICALS
Convenient MD, located at 125 Indian Rock Road in Windham, N.H. which is at the Intersection of Rt. 111 and 111A at Exit 3 off Rt. 93, has donated vouchers for Free Flu Shots and if needed Free Sports Physicals for students who attend Pelham High School. The vouchers are located in the school nurse’s office. Please stop by to get one if interested or call the school nurse to request one at 635-6906.

NOTICE TO PARENTS ABOUT FREE VACCINES FOR CHILDREN:
The State of New Hampshire provides free vaccine for children up to age 19 to all Pediatrician Practices and Other Health Providers that service children and adolescents in the State of New Hampshire. You should only have to pay a co-payment if that applies with your insurance. If you do not have insurance you can immunize your child at Nashua Health Department, 18 Mulberry St. in Nashua, N. H.

Please call 589-4500 for an appointment. Clinic hours are Tuesdays from 4 PM to 7 PM and Fridays 8:30 AM to 10:30 AM. You will need a copy of your child's Immunization Record from their doctor or school nurse or a letter from school saying which vaccines your child needs. School nurse's and doctor's offices can fax this information directly to the clinic at 594-3323 or you can bring a copy to the clinic at the time of your appointment.

Ask for specific directions to the clinic and where to park when you call to schedule a clinic visit.

A donation of $10 is requested for each shot but not mandatory if you cannot afford to pay.

***The clinic can provide all childhood vaccines including a Tdap which is now required as a one time booster for children age 11 and older, the HPV (Gardasil 3 shot series) for females and males 12 and older, the Varivax for chicken pox, the Menactra for meningitis, and Hepatitis A.
