Reporting Form for COVID-19 during the Holiday Break

To help us plan appropriately, please complete this reporting form if you have a child in the Pelham School District who is:

1. experiencing symptoms consistent with COVID-19, or

2. has tested positive for COVID-19, or 

3. has been exposed to someone in their household who has tested positive for the virus.

Please complete one form per student. This is for events between December 23, 2021 and January 2, 2022. If your child meets any of these conditions, please keep them home on Monday January 3. Our teams, including school administration and nurses, will review the forms, contact you to gather additional information, and determine appropriate next steps.
