Immunization Information

All students need a TdaP (tetanus/pertussis) booster if they are going into 7th grade by the first day of school, if this is not done your child may be excluded from school, this is for your child's safety as well as the safety of other children in the building. Please send in updated immunization information as soon as possible, as well as physicals. If you have an appointment after the first day of school please let me know.

Children can be excluded from school, per the state, if immunizations are not up to date, there is no appointment date set up and there is no record of this in the school. If there is a problem you can contact the state department of immunizations and vaccinations or check on their web site: at dhhs.nh.gov/dphs/immunization/schools.htm or dhhs.nh.gov/dphs/immunization/exemptions.htm. The only way a child is excused is by a religious or medical exemption, that has been notarized and is on file in the child's record. All immunizations must be in English and include full dates. Requirements do change and different states have different recommendations.

Any questions please contact Mrs. Levine.

I have been a nurse for 43 years. I have worked in hospitals doing mostly Pediatric nursing. As a nurse I also worked as a camp nurse during the summer. I have been with the Pelham School district since 1985, but since 1994 full time. This is my 28th year in the Pelham schools either full time or substituting in the classroom. I lived in Pelham for 39 years with my husband, and 4 children, who all attended and graduated from Pelham schools.
